ATHLETE MEMBERSHIP AGREEMENT
USA GYMNASTICS (THE GOVERNING BODY FOR GYMNASTICS IN THE UNITED STATES) SUGGESTS THAT ALL ATHLETIC FACILITIES REQUIRE AN ATHLETE MEMBERSHIP AGREEMENT FORM FOR EACH PARTICIPANT. THEREFORE, WE REQUIRE EACH STUDENT TO HAVE A SIGNED FORM ON FILE PRIOR TO THE START OF THE FIRST CLASS. (PLEASE READ THE FOLLOWING CAREFULLY AND CLICK "I AGREE" BELOW. NOTE: PARENT AGREES IF STUDENT IS UNDER 18) AGREEMENT IN CONSIDERATION OF MY PARTICIPATION IN TRIUMPH RHYTHMIC GYMNASTICS ACADEMY CLASSES, EVENTS, AND ACTIVITIES, I AGREE TO BE BOUND BY EACH OF THE FOLLOWING: 1. ELIGIBILITY: I AGREE TO COMPLY WITH THE RULES OF TRIUMPH RHYTHMIC GYMNASTICS ACADEMY. 2. READINESS TO PARTICIPATE: I WILL ONLY PARTICIPATE IN THOSE TRIUMPH RHYTHMIC GYMNASTICS ACADEMY CLASSES, EVENTS, COMPETITIONS, AND ACTIVITIES FOR WHICH I BELIEVE I AM PHYSICALLY AND PSYCHOLOGICALLY PREPARED. I WILL ONLY PERFORM THOSE EXERCISES BY MYSELF IN WHICH I FEEL CONFIDENT TO DO SO. 3. AS STATED IN THE ACKNOWLEDGEMENT OF RISK, I AM AWARE OF THE RISKS INVOLVED IN ACTIVITIES CONDUCTED AT TRIUMPH RHYTHMIC GYMNASTICS ACADEMY AND I AGREE THAT TRIUMPH RHYTHMIC GYMNASTICS ACADEMY AND THE SPONSOR OF ANY TRIUMPH RHYTHMIC GYMNASTICS ACADEMY EVENT, ALONG WITH THE EMPLOYEES, AGENTS, OFFICERS, AND DIRECTORS OF THESE ORGANIZATIONS SHALL NOT BE LIABLE FOR ANY LOSSES OR DAMAGES OCCURRING AS A RESULT OF MY PARTICIPATION IN THE EVENT. 4. IN THE EVENT THAT I, OR ANYONE ELSE ON BEHALF OF MYSELF, MY CHILD/CHILDREN, BRING SUIT AGAINST TRIUMPH RHYTHMIC GYMNASTICS ACADEMY ALONG WITH THE EMPLOYEES, AGENTS, OFFICERS, AND DIRECTORS OF TRIUMPH RHYTHMIC GYMNASTICS ACADEMY AND SAID SUIT DOES NOT RESULT IN JUDGEMENT AGAINST TRIUMPH RHYTHMIC GYMNASTICS ACADEMY ALONG WITH THE EMPLOYEES, AGENTS, OFFICERS, AND DIRECTORS OF TRIUMPH RHYTHMIC GYMNASTICS ACADEMY, I HEREBY AGREE TO BE LIABLE FOR ALL ATTORNEYS FEES AND COSTS INCURRED BY TRIUMPH RHYTHMIC GYMNASTICS ACADEMY ALONG WITH THE EMPLOYEES, AGENTS, OFFICERS, AND DIRECTORS OF TRIUMPH RHYTHMIC GYMNASTICS ACADEMY, AS A RESULT OF SAID LAWSUIT.

MEDICAL EVALUATION INFORMATION
TRIUMPH RHYTHMIC GYMNASTICS ACADEMY SUGGESTS THAT PARTICIPANTS OBTAIN A MEDICAL EVALUATION FROM A PHYSICIAN BEFORE ACTUAL PARTICIPATION IN ANY OF ITS' ACTIVITIES. I EITHER DECLINE TO OBTAIN A MEDICAL EVALUATION FROM A PHYSICIAN PRIOR TO PARTICIPATION IN ANY OF TRIUMPH RHYTHMIC GYMNASTICS ACADEMY ACTIVITIES OR I WILL PROVIDE TRIUMPH RHYTHMIC GYMNASTICS ACADEMY WITH A MEDICAL EVALUATION AND RELEASE FORM FROM THE PHYSICIAN WITHIN TWO WEEKS OF REGISTERING. ADDITIONALLY, TRIUMPH RHYTHMIC GYMNASTICS ACADEMY RECOMMENDS THAT PARENTS VISIT THE CDC WEBSITE AT 
http://www.cdc.gov/concussion/headsup/online_training.html FOR INFORMATION ON CONCUSSIONS AND WHAT TO WATCH FOR IF A STUDENT HAS ANY LEVEL OF INJURY TO THEIR HEAD. TRIUMPH RHYTHMIC GYMNASTICS ACADEMY COACHES ARE PARTICIPATING IN THE CDC ONLINE TRAINING FOR COACHES.

TRIUMPH RHYTHMIC GYMNASTICS ACADEMY POLICY CHECKLIST
Please read each section indicating you are aware of and agree to follow our policies, then click "I Agree" at the end of the form: Fees: All Gymnastics classes are billed as monthly autopay on the 1st of each month (summer camp can be billed on daily or weekly bases). Updates to flyers and website (time changes, price changes, etc) are by program and must be noted before signing. Change of payment: If you would like to use a different form of payment than what is on file, change it on the member portal before the 1st of the month. Pro-rating: We pro-rate for first time students only. Make-up Policy: Triumph Rhythmic Gymnastics Academy does not limit make ups to enrolled students whose tuition is current and up to date where space is available. Students who have dropped their enrollment are no longer eligible to do make ups. Make-ups are a courtesy and we do our best to accommodate all students. In order to make-up a class, you must call ahead and schedule the make-up through the office or sign up in the gym. This allows the staff to check for class and time availability. Make ups are for absences and emergency gym closings only, not scheduled gym closings (see flyers/handouts). Attire: Please make sure that your child is properly dressed. Dress Attire: tight fitting top and bottom (black only for levels 5 and up, xcel), white socks or half-toe shoes, hair pulled back in a bun, and no jewelry. Sibling Discount: A 10% discount will be given off the additional student's tuition (lesser fees). Multi class discount of 10% will be given on additional classes by the same student (only applies to summer camp). Hurricane Weather: In case of a hurricane, we will follow the same closures as Palm Beach County Public Schools. Civic Closure: Should the facility be closed by Civic order (ie: pandemic, weather system, etc), the following policies will be in effect for all classes: 1. Should the closure be mid-month, NO refunds will be issued for the remainder of the month. Make-ups will be provided upon re-opening, as long as the student remains enrolled and paid. 2. If the closure continues into the next month, class tuitions will be charged. Make ups will be recorded and provided upon opening as long as the student remains enrolled and paid. Dropping a class: If for some reason you need to drop a class, simply fill out a drop form on or before the 15th of the previous month. Pets: Due to the fact that many people have allergies, only certified service animals are permitted in the building. Parent's Responsibilities: Parents and children who arrive at the gym early must remain in the lobby until the instructor calls for the class. No one but the enrolled student is permitted in the gym area. Siblings not enrolled in a class must remain with a parent, in the lobby, at all times. Your child counts on you to get them to class on time. Remember - warm-up is a crucial element of their class. Pick up your child promptly. Please inform us if you have an emergency or will be late. Safety: Although we at Triumph Rhythmic Gymnastics Academy promote learning and fun, safety is of our utmost concern. Please watch your child(ren) carefully in the parking lot. Please drive slowly and watch for other children in the parking lot. Keep all children in waiting area before class. Do not allow children to climb on anything in lobby area. Keep siblings/friends out of gym during class time. Remember that parents are not permitted in gym area except in Parent/child classes. Photos/Videos: Occasionally Triumph Rhythmic Gymnastics Academy would like to take videos/pictures of our students for use on our website or Facebook/Instagram page. Free trial classes for Gymnastics given to students who are new to a program and are only given once within that program. (pay by day classes not included). Any spectators must have a signed waiver on file prior to entering the gym. We will issue a credit to your gym account only with the POSSIBLE exception of a medical emergency.

ANNUAL ADMINISTRATION FEE
An annual administration fee of $40.00 per child is also required and will be charged at time of registration for new students. Annual registration fees will be automatically billed on each anniversary of first class registration. If you are registering for camp, the $40 annual fee will automatically be added to your account but will be adjusted by our staff prior to your payment, By using this online service YOU AGREE to all terms and conditions applicable to your enrollment!

ACKNOWLEGEMENT OF RISK
NOTICE TO MINOR CHILD'S NATURAL GUARDIAN USA GYMNASTICS (THE GOVERNING BODY FOR GYMNASTICS IN THE UNITED STATES) SUGGESTS THAT ALL ATHLETIC FACILITIES REQUIRE AN ACKNOWLEDGEMENT OF RISK FORM FOR EACH PARTICIPANT. THEREFORE, WE REQUIRE EACH STUDENT TO HAVE A SIGNED FORM ON FILE PRIOR TO THE START OF THE FIRST CLASS.

I further acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation and exposure. I UNDERSTAND THAT THIS IS A SPORTS FACILITY AND UNEVEN SURFACES ARE INHERENT TO SUCH FACILITIES. IF, AT ANY TIME, I AM PERMITTED TO ENTER ANY OF THE SPORTS AREAS I WILL ENTER AT MY OWN RISK AND I WILL WATCH FOR SUCH UNEVEN SURFACES. I FURTHER UNDERSTAND THAT TRIUMPH RHYTHMIC GYMNASTICS ACADEMY DOES NOT PERMIT ADULTS OR NON-ENROLLED PARTICIPANTS TO BE ON OR UTILIZE ANY SPORTS EQUIPMENT OR MATS. BY CLICKING "I AGREE" BELOW, YOU ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTAND THE RISKS INVOLVED IN PARTICIPATION OF ACTIVITIES TRIUMPH RHYTHMIC GYMNASTICS ACADEMY. IF THE PARTICIPANT IS MY CHILD I WILL MAKE SURE THAT MY CHILD HAS BEEN INFORMED.

Camp Policy Agreement-Effective Summer 2026

Age: Camp available for children Ages 4+ only. 

Time: Half Day Camp is 9:00am - 12:00pm 

           Full Day Camp is 9 am - 4:00pm

Camp Fees: *Camp prices listed are for enrollments booked/paid for at least 24 hours in advance-less than 24 hours in advance add $10 per day. Camp enrollments made after the Friday prior are charged at enrollment.  *Pre-enrollment (week or more before) 10% deposit due at enrollment-non-refundable. Balance may be paid at enrollment but MUST be paid by Friday of prior week. Unpaid balance will forfeit deposit and students will be dropped from upcoming week. Full price will due upon re-enrollment-space permitting.  *Discounted Weekly fees: M-F of same week-no carryovers, no make ups.

CAMP FEES ARE NON-REFUNDABLE. A GYM CREDIT MAY BE CONSIDERED FOR MEDICAL EMERGENCIES WITH A DOCTOR'S NOTE!  CAMP DAYS MAY BE RE-SCHEDULED WITH A MINIMUM OF 24 HOURS NOTICE.

Discounts: 10% sibling discount on LOWER tuition amount. 10% discount on multiple weeks. 10% discount on Bring a Friend.
